Manage the intake process through which organizations collect demographic, social and clinical data, consent forms, insurance, payments, and other key pieces of information from new and returning individuals receiving services. That’s intake management!
Capture and follow referrals from multiple sources without losing sight of any opportunity. Referral information is added in the PrecisionCare software platform using a list of referral sources available to the agency, which include addresses, phone numbers, email, and type of sources. PrecisionCare provides an array of reporting capabilities for tracking referrals, such as referral reports, follow up reports, calendar reports and more.
Keep personal, cultural, and social details organized for holistic care. PrecisionCare captures several different types of general information on individual demographics, such as age, birthday, gender, and ethnicity/race. Information about an individual is entered only once into care management software, then shared throughout the system on required forms. Demographic information will automatically print to required forms and is also tied into areas of the system.
Record vocational history, job placements, and outcomes to support long-term independence. PrecisionCare’s employment tracking function supports Supported Employment for Intensive and Extended SEMP. When these supported employment services are created, they are identified as Intensive or Extended, which will then dictate the documentation requirements for these services. Employment status, placement and employers are all tracked with the appropriate history within our care management system.
Centralize essential contacts for seamless communication and coordination. PrecisionCare’s contact management captures each individual’s contacts, such as their family, guardians, personal representatives, natural supports, care managers, and staff.
Manage client authorizations and disclosure forms securely. An individual receiving services may approve a consent for release authorization for disclosure of their personal health information (PHI). PrecisionCare supports a number of functions and customizations around consent. We enable users to create Consent for Release forms directly in the system. These forms and reports can be created dynamically by users or provided by PrecisionCare. The information that is entered on any consent form is saved directly in our care management database, digested for authenticity, and can be displayed or presented to authorized users. PrecisionCare provides the ability to create, populate, send externally for signatures, receive signed forms, store them within an individual’s record, and update an individual’s status to record fulfillment of consent requirements. PrecisionCare supports multipurpose consents that require signatures of the staff completing the consent for release form, and the authorized individual receiving services, or their legal guardian or personal representative. PrecisionCare gives users the ability to identify the authorizing party. If the authorizing party is someone other than the individual, they must then be a legal guardian or personal representative.
Build and monitor treatment plans aligned with clinical goals and client needs. On the PrecisionCare platform, treatment planning feature allows for a customizable plan to be created for an individual, which is a living document that follows them throughout the care provided to them to reflect progress towards their goals/supports as needed. It contains measurable objectives and a timeline for treatment progress.
Create personalized care plans with regular progress checkpoints. Service plans are developed by care service providers, with input from the individual served, to clarify what services and support they will provide and the priority of those services. These service plans often have very specific goals or objectives that the individual is expected to achieve as a result of receiving the services. PrecisionCare offers care providers the ability to have regular reviews of service plans (monthly, quarterly, semi-annually) to review goals and support provided to each participant. The Reviews area is also customizable.
Develop and monitor individualized intervention strategies. PrecisionCare allows for the creation of custom Behavior Support Plans for various behavioral needs. Users can track the identified target behaviors from the Behavior Support Plans created in our system.
Track personal milestones and program outcomes with measurable insights.With PrecisionCare EHR software, users have the ability to provide details (ex: status, attained or unsuccessful, how long the goal was active, how many prompts, etc.) of each goal being tracked.
Document daily activities, both individual and group, in real time. PrecisionCare’s solution provides the capability for users to record and track a variety of Progress Notes by program, location, and service type. PrecisionCare offers fully customizable notes that can have unique fields, functionality, and security. Notes may require Face-to-face meetings, specific location requirements, travel information and much more. Progress notes can be amended with tracking capability. PrecisionCare also offers the ability to create Group Notes to capture shared activities quickly for a group of participants.
Summarize progress, compliance updates, and key milestones every month. PrecisionCare offers the ability to create monthly notes to review goals and supports provided to each participant.
Record, monitor, and resolve incidents with full accountability. PrecisionCare’s incident management and investigation system helps investigators create a final report that reflects a thorough and fair investigation and produces a written investigative report in a format that will help others understand what happened and why. Classifications are defined by the justice center and the agency, and the system features dynamic fields based on the classifications chosen. Identify and track recommendations for improvement and use the summary and trend reports to help anticipate serious incidents before they occur. Within PrecisionCare you can manage all aspects of incident management including incident reviews, receive reminders for follow-up, and generate redacted reports. PrecisionCare also allows you to create customized forms and reports, and track individual and group incidence, investigations, meeting minutes and trending reports.
Manage service approvals, renewals, and coverage limits with confidence. With our care management system, participants can have their service deliveries capped and counted via Authorization Tracking. This is especially useful for when a service has a limited number of services approved or limited amount of time approved.
Empower individuals and families to actively manage their service plans. Self-direction gives an individual the flexibility to choose the mix of support and services that are right for them, so they can live the life they want. With self-directed services, they choose their services, the staff, and the organizations that provide them, and a schedule that works best for them. They have more control over the funding used to buy services. PrecisionCare supports each aspect of Self-Directed Services. Everything from the budget to family expenses, service delivery, staff time and billing. Providers and advocates access our system securely via our Individual/Family Portal.

PrecisionCare offers a comprehensive assessment building feature that allows users to create an unlimited number of customized forms. A System Administrator can be trained on how to create any assessment or form desired and also have the ability to use their own forms or those created by other agencies.

Our EHR software solution offers a robust medical appointment tracking/management feature that enables users to create and track future appointments for individuals while maintaining data on past appointments. The appointment can be scheduled for any time in the future, and a reoccurrence menu may be used to schedule multiple appointments at one time. Users may view and print a calendar of each scheduled appointment and run reports that summarize all medical appointments.
Organize prescriptions, refills, and histories in one place. PrecisionCare’s platform offers a highly customizable medical management/tracking area, which creates a comprehensive paperless medical file.
Log treatments, follow-ups, and key health events. Administrators can create and customize unique medical tracking forms within PrecisionCare’s care solution. They also can receive alerts for specific events occurring, or for the lack of a specific event occurring.
Automate medication administration and sync seamlessly with pharmacies.
Connect with regional health information exchanges to create a complete care record. Among other care management benefits, PrecisionCare also offers electronic prescribing of medications (E-Prescribing) and controlled substances (EPCS), eligibility checks, and formulary verification. PrecisionCare’s electronic medication administration record tool allows providers to prescribe, review and order medications. PrecisionCare will interface with the pharmacy, which will import the medications and schedule directly to eMAR.
Access standardized diagnosis codes and references for accurate clinical documentation. The Centers for Disease Control and Prevention (CDC) developed and maintains the ICD-10-CM (Diagnoses) code set. These codes describe an individual’s disease or medical conditions. PrecisionCare provides a comprehensive library of these codes and their descriptions. Authorized users have access to a diagnosis lookup tool, which allows them to enter the diagnosis code for an individual.
Manage billing and coding efficiently with integrated CPT libraries. The Current Procedural Terminology (CPT) codes offer health care professionals a uniform language for coding medical services and procedures to streamline reporting, increase accuracy and efficiency. With PrecisionCare’s CPT Code Management, clinics can manage and customize the CPT codes used.
Collaboration and Communication

Give clients and families secure access to updates, notes, and care plans. Our individual and family portal provides support for individuals and their families that connects their treatment and care with user-friendly, secure access. It contains a modified menu and provides read-only access to an individual’s documents. It also has an electronic signature screen for plans to be signed with any device using a finger or stylus. Family and treatment providers can use this when logged on or when presented with the document on a mobile device.
Communicate with HIPAA-compliant, encrypted messaging. PrecisionCare’s offers secure messaging communication that functions like email and can be sent to specific users of the system or portal users, with or without attachments. Only authorized users can log into our care platform, or the portal can access the messages.
Receive real-time notifications for incidents, tasks, and urgent updates. PrecisionCare’s Alerts feature is based upon an action and triggered when something has happened, such as an appointment.
Keep a full record of interactions between staff, providers, and families. PrecisionCare features CareLog Communication Logs that can be used to communicate between users. The CareLog can have categorized log entries, attachments, photos and more, all contributing to enhanced and easy communication between users.
Simplify approvals with secure e-signatures. PrecisionCare uses digital signatures on signed documents that maintain not only the individual that signs the documents, but the actual content (integrity) of the document is also secured within our care system database. Digital signatures eliminate the need to print out the case record, any supporting notes and any data that has been collected as part of service delivery. Digital signatures are stronger than electronic signatures because they validate the integrity of the signed document. Electronic signatures are used to allow external signing of documents from guardians to individuals. PrecisionCare pioneered the use of electronic signatures in the IDD community in 2009. Through an agency’s electronic signature policy, plan authors and approvers are captured. The electronic signature policy defines how e-signatures are used within an agency. In an audit this is referred to regarding the contemporaneous timeframe that an agency sets in order to give staff reasonable lead time to enter their documentation. The policy also identifies the strength of the signing password that will be used. PrecisionCare enforces the strength and valid period of the signing password, as defined by the agency.
Deliver remote care through integrated video conferencing tools. PrecisionCare’s secure telehealth feature gives agencies a user-friendly way to provide virtual services, making care easier and more accessible. No downloads or special equipment are required – just a simple connection through email, text, or Messenger.

Customized data collection areas are available to enable unique items to be collected during this process.
Track client participation across programs and activities. PrecisionCare provides the ability to track participant attendance (specific time) during a program. The attendance area also tracks any associated transportation to and from the program for each participant.
Create on-demand reports without waiting on IT. Ad hoc reporting is a business intelligence process used to quickly create reports on an as-needed basis. PrecisionCare has extremely broad, comprehensive reporting capabilities, and we offer an extensive library of reports and queries that are easily customized. Besides the standard reports that are required for service documentation, PrecisionCare also offers a variety of ad hoc reports. Our summary reports are user configurable. Each one is based on key areas of the software and provides the user with a text view of data summarized as needed.

Our care EHR software platform offers a data exporting feature that allows the agency to request a unique report, which is delivered to them in spreadsheet format. An agency can also have VPN access to their live database to create reports, as well as PrecisionCare’s data extraction and analytics tools to support KPI and predictive analytics reporting.

Maintain a complete activity log for transparency and compliance. Audit logs and reports may be run at any time for any timeframe, date, or for specific actions. These audit tracking actions range from navigational cues, like editing/modifying, adding, deleting, and querying data. This report gives a high-level overview of what a specified user (or list of users) has done during the time frame. This report is typically used for training purposes, but also for auditing data changes when needed.

PrecisionCare supports intermediate care facilities (ICF), which are long-term care facilities that provide nursing and supportive care to residents on a non-continuous skilled nursing care basis, under a physician’s direction. ICFs are designed to provide custodial care for those who are unable to care for themselves because of mental disability or declining health.
Manage staff roles, profiles, and certifications. PrecisionCare offers an internal identify management feature via the Human Resources Management component of the software. This allows a System Administrator to modify user accounts and give access to specific areas of the system. Almost all areas of PrecisionCare are controlled via access in the Human Resources area.
Track staff training, credentials, and required courses. Our care management software has a comprehensive staff training management component where all training is tracked. Rules can be applied that would prevent notes from being certified or billed in the absence of or upon expiration of required training.
Organize donor databases and fundraising campaigns. The Development area of PrecisionCare has fundraising tools that organizations can use for tracking public relations, fundraising and other development activities. This customizable module tracks each aspect of fund raising from events to contributors. As part of the common database, consumer contact, staff and treatment provider information may be shared throughout the entire system. PrecisionCare enables organizations to effectively manage all of their developmental efforts.
Verify visits electronically for compliance with state and federal mandates. Our web-based EHR platform provides Electronic Visit Verification (EVV), an electronic system that verifies when provider visits occur and captures the date, time and location of the visit, the person who received the services, the person who provided the services, and the services provided. The goals of EVV are to ensure timely service delivery, including real-time service gap reporting and monitoring, reduce the administrative burden associated with paper timesheet processing, and generate cost savings from the prevention of fraud, waste, and abuse.
Centralize deadlines, events, and reminders in one calendar. PrecisionCare’s software robust calendar feature contains date sensitive items such as appointments that are coming due or past due. It has a monthly, weekly, or daily view and can be broken down by program to identify if appointments have been received.
